Click on the class date (below) of the class you attended to get to the online class evaluation on Survey Monkey. Completion of a class evaluation is required in order to receive contact hours.

When TCHP receives your completed evaluation and has verified your attendance at class, you will be issued a Certificate of Attendance. This is not an automatic process. Please allow up to 2 weeks to process your Certificate of Attendance.
